If the Nikon Super CoolScan 5000 or any of the accessories must be repaired by Nikon technical support, complete the following actions:

1. Double-check to make sure machine can't be fixed in house.
2. Call Nikon Techincal Support at 800-645-6678.
3. Fill out a repair form found at nikonusa.com/service. 
4. Submit the form and print out two copies.
5. Make sure to only send the item that needs to be fixed. Remove all adapters, cords, etc. 
6. Pack the item in a large box by first placing the item in a plastic bag and making sure that it is surrounded by quality packing material. Also include one of the repair forms for their use. 
7. Print out shipping label and secure it to the box. 
8. Keep the additional form, making sure to note the confirmation number of the service requested and the shipping tracking number.
